your blood glucose monitor (if you have one)
diabetes management diary (if you have one)
medicare card
DVA card (if you have one)
private insurance card (if you have one)
Referral types
Self- referrals – you can refer yourself to us (self-founded (check with your private insurer for eligibility))
GP referrals – an Enhanced Primary Care Plan referral (ECP) through your General Practitioner, may enable you to claim a Medicare rebate.
Endocrine referral – referral by your endocrinologist (self-founded (check with your private insurer for eligibility))

When you attend Glucose Compass Diabetes Service the Credentialed Diabetes Educator (CDE) will complete a comprehensive diabetes assessment, set goals for your specific need, act as an advocate when needed and implement a mutually agreed care plan with you. CDEs can assist clients who have prediabetes, type 1 diabetes, type 2 diabetes or gestational diabetes with diabetes self-management and all other types of diabetes.
